Summer Paperwork
1. Enrollment Form
Time to complete 5-10 minutes, if you have your documents ready to upload.
Once in the form, make sure to click that you are enrolling in the JEA Program.
It will customize the form for JEA.
You will need to upload one verification document. Take a picture of one document from the list below before filling out the form. Also, if you give us school documents consent, it will ask for an additional screenshot of your report card.
Proof of Address:
- Copy of the student’s report card or transcript with the home address listed on it.
- BPS ASPEN student profiles, which can be found under my info. Make sure the picture includes the youth’s name, school name, home address, and current school year.
- Copy of utility bill
- Copy of bank statement
- A current lease, Section 8 agreement, or BPS residency affidavit
- State ID or Driver’s License
2. Media Release Form
Time to complete 1-2 minutes.
3. W-4 Form: Federal Taxes
Time to complete form 7-10 minutes.

4. M-4 Form: State Taxes
Time to complete form 7-10 minutes.
Tip one: Question 1, write 1.
Tip two: Question 4, add up all your exemptions.
Tip three: Question 5, we recommend checking box D if it applies to you.

5. I-9 Form
Time to complete form & scheduling link 10-20 minutes.
Tip one: For the check box section, if you select 3 or 4 make sure to fill in the other boxes that are near 3 and 4.
Tip two: Take a picture of your I-9 verification documents before filling out the form.
- Either JUST ONE from list A, if you have it.
- If you don’t have one from list A, you will need ONE from list B AND list C.

We are required to see the documents (in-person) that you uploaded for this form. Please book a time to meet with Eva to complete your I-9:
- At this in-person appointment, bring the original verification documents you uploaded for your I-9.
- We may ask you to bring other documents with you in case we are missing anything from your other summer forms.
- Directions to our building will be included in the appointment invitation and reminders. A quick building tour can happen if you would like it.
- This step needs to be done to fully complete the I-9.
- If you cannot find a time below that works with your schedule, please email recruitment@hydesquare.org to request another time.
6. Work Permit Application: for Youth ages 14-17
You can only receive your Work Permit by submitting a completed Work Permit APPLICATION.
Work Permit APPLICATION instructions are below. Read carefully.
This is the only document that you can turn in past your summer paperwork deadline. Deadline extensions are available only if you are waiting on the doctor’s or your school office. You must communicate with your HSTF staff member if you need a deadline extension. If you do not move this process along, you risk being moved to the waitlist.
Step one: Time to complete 2-5 minutes
Once you submit the Work Permit APPLICATION below, we will email you a PDF copy. Use that copy to complete the next steps. If you need a printed version, email us back to request one.

Step two: If you are 14 or 15 do this extra step.
If you are 16 or older, skip to Step three.
Time to complete is 3 business days to 4 weeks, depending on your doctor.

- 14-and 15-year-olds need a doctor’s signature on the second page of the Work Permit APPLICATION.
- Call your doctor to find out about their work permit application process. Ask if you can send the Work Permit APPLICATION PDF via email or if they need to sign a printed copy. Your doctor may require you to make an appointment to get their signature.
- Once your doctor signs it, go to Step three.
- 14-and 15-year-olds need a doctor’s signature on the second page of the Work Permit APPLICATION.

Step three: Time to complete, once submitted to your school, normally 1-3 school days.

- Once all signatures are collected on your Work Permit APPLICATION, turn in your APPLICATION to your school’s office and ask them (kindly) to process your Work Permit APPLICATION.
- It might take a day or two to process, but continue checking in to see if they have your Work Permit.

Step four: The time to complete is up to you could be done in 2 minutes digitally or submit your Work Permit directly to Eva at HSTF.

7. Direct Deposit (optional)
Time to complete 10-12 minutes.
If you want your checks directly deposited into a bank account, complete this form with the youth’s information, signature, date, and upload the supporting documents.
Tip one: The Account Holder adds their bank information into the Direct Deposit section.
Tip two: The Worker’s Signature is the youth’s, and the Account Holder’s Signature is whoever owns the account.
Tip three: The form will ask you to upload a verification document. Upload one of the following documents:
- Voided Check
- Bank letter specification sheet
- Bank Direct Deposit letter (you can often find this in an online search or on your online banking app)

Congratulations, you did it!
Your summer paperwork process is officially complete when we confirm that you are approved for the summer. If our HR department is missing anything, we will reach out to both you and your parent from your paperwork email; keep an eye on it.
Before summer starts, you will receive two invitations, one for our JEA summer orientation and another to get on our texting platform, REMIND. In addition to these updates, you may also be chosen to be a Successlink Youth, which means some additional paperwork may be needed, and often we will get at your I-9 appointment. We will let you know if you have been selected.
