LSJS Youth Organizing: Admission paperwork
1. Enrollment Form
Once in the form, make sure to click that you are enrolling in the LSJS Program.
It will customize the form for LSJS.
It will ask you to upload one verification document. Take a picture of one document from the list below before filling out the form.
Proof of Address:
Copy of student’s report card with home address listed
Copy of utility bill
Copy of bank statement
BPS student ID
State ID or Driver’s License
A Letter from an Approved Government Agency* dated within the past 60 days
A current lease, Section 8 agreement, or BPS residency affidavit
School record or letter with home address listed out it.
