Caminos: Admission paperwork

These forms will require signatures from participants and or parent/guardians if the participant is under the age of 18.

Once in the form, make sure to click that you are enrolling in the Caminos Program.

It will customize the form for Caminos.

It will ask you to upload one verification document. Take a picture of one document from the list below before filling out the form. Also, if you give us school documents consent, it will ask for an additional screenshot of your report card or transcript.

Proof of Address:

Copy of student’s report card with home address listed

Copy of utility bill

Copy of bank statement

BPS student ID

State ID or Driver’s License

A Letter from an Approved Government Agency* dated within the past 60 days

A current lease, Section 8 agreement, or BPS residency affidavit

School record or letter with home address listed out.

 We will be sending you an invitation from Remind, our texting platform, where all Education staff can share updates with you. We are looking forward to meeting you and welcome to our HSTF community!
